About Your Visit
Our doctors and professional staff look forward to meeting you! We respect your time and would like to make your visit to our office as efficient as possible.
If this is your first visit to our office, you will receive a letter in the mail with new patient information forms. For your convenience, you may also download a copy of the forms:
Other items to bring with you may include:
- Insurance cards
- Referral authorizations - if necessary
- List of your current medications
- Pertinent records from your physician
- CT scans or MRI films
- Any recent audiograms - hearing tests
- Past surgical history
Financial Policy
We will collect your deductible, co-payment, or charges for non-covered services at the time of your visit. Even though an insurance claim may be filed, you are responsible for the total amount on your account. if you have a balance after an insurance payment from a previous service, we will also ask for that payment. We accept cash, checks, debit, and credit cards.
Cancellation Policy
If you are unable to keep your appointment, we ask the courtesy of a phone call to (810) 694-0600 as early as possible, so we are able to offer that time to another patient.

Privacy
